Transportation Form
To prepare for your child’s transportation needs for the upcoming school year, please complete the transportation form by clicking here (open and then scroll down to the Elementary Transportation Authorization Form). Please complete as soon as possible. Every student will need a newly completed transportation form each school year.
This form should only be filled out ONCE per student. After completing the online form please bring it to school on orientation day. Please call the front office at (678) 344-6100 with any questions.
Transportation Changes
The best transportation plan for students is a consistent plan! Students should travel home the same way each day. We are aware that changes are necessary and should be as infrequent as possible. To make a change to your student's transportation form, we require a hand written note by the parent/guardian. The note should be sent with your child to be given to their teacher in the morning. We are unable to accept transportation changes after 11:30 a.m. Please use the template below to provide the necessary information to the school when making a transportation change.
